Our Commitment

Declaration of Transparency

Florida Accountable Care Services is committed to transparency, accountability, and the highest standards of care for Medicare beneficiaries across Florida.

Our Pledge to Patients & Partners

Florida Accountable Care Services (FACS) participates in the Medicare Shared Savings Program (MSSP), a voluntary program that encourages groups of doctors, hospitals, and other health care providers to come together to give coordinated, high-quality care to Medicare patients.

As part of our participation in the MSSP, FACS is required to meet certain transparency requirements. This page describes how we fulfill those obligations and our broader commitment to open, accountable healthcare delivery.

Commitment to Quality

FACS is committed to delivering high-quality, coordinated care to Medicare Fee-for-Service patients. We continuously monitor and improve our performance against CMS quality benchmarks.

Public Reporting

As required by the Medicare Shared Savings Program, FACS publicly reports performance data annually. This information is available to all Medicare beneficiaries and the general public.

Beneficiary Rights

Medicare beneficiaries retain all their rights under Medicare. Participation in FACS does not restrict a beneficiary's ability to see any Medicare-participating provider.

Governance & Accountability

FACS maintains a governing body with meaningful participation from ACO professionals and Medicare beneficiaries, ensuring accountability to the patients and communities we serve.

Notice to Medicare Beneficiaries

Your doctor or other health care provider may be sharing your health information with Florida Accountable Care Services (FACS), a Medicare Accountable Care Organization (ACO). ACOs are groups of doctors, hospitals, and other health care providers who work together to give you better, more coordinated care.

Your Medicare benefits are not changing. You still have the right to use any doctor, hospital, or other provider that accepts Medicare, at any time. Your costs under Medicare are not changing. You are not required to use providers in the FACS network.

You may opt out of sharing your health information with FACS at any time by calling 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048. If you opt out, your health information will not be shared with FACS for purposes of this program.
